*Job Summary*:
SkyBridge Healthcare is seeking a Travel *Home Health Registered Nurse (RN)* *Case Manager* in *White River Junction, Vermont!*
*Details*
*Duration: *16 weeks
*Schedule*: M-F 8AM - 4:30PM, 40 hours scheduled weekly
*On Call/Weekend: *2 weekend days every 4 weeks, with days off during the week to compensate
*EMR: *EPIC
*Guaranteed Hours: *32 hours weekly guarantee
*Requirements*
* Experience as a Home Health Registered Nurse
* *Current Registered Nurse (RN) license in good standing.*
* *Strong critical thinking skills* and the ability to make sound clinical decisions independently.
* *Excellent communication skills* to effectively interact with patients, families, and healthcare team members.
* *Commitment to maintaining high standards* of patient care and safety.
* *Maintains a positive attitude*, ready to assist patients and the agency, and is open to learning.
* *Punctual and engaged *in weekly meetings, including virtual attendance with camera on if needed.
* *Current BLS Certification*
* *Current Driver's License + vehicle insurance*
* *Access to reliable transportation*
