*Position Details:*
* Full-time Hospice RN Case Manager position
* Schedule: Monday thru Friday, 8a-5p
* Available territory openings: Delaware County, Madison County, and surrounding communites
* RN Case Manager typically assigned 1 county to cover.

*Job Functions:*
* Performs physical and psychosocial assessment on all hospice patients.
* Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
* Responds to the physical, mental, social and emotional needs of terminally ill patients and their families.
* Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary.
* Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate.
* Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc.
*Requirements*
*Experience:*
* One year Hospice experience preferred, but not necessary
* 1 year of RN experience required
*Credentials:*
* Licensed Registered Nurse in State of Indiana
* Current driver's license
* Must have reliable transportation and insurance

Pay: $72,000.00 - $82,000.00 per year
