Job Overview Onsite Health Coach at Elevance Health focuses on providing health coaching to members across all brands with chronic diseases. This field‑based role requires traveling to client sites, occasional office attendance, and a 5‑day schedule (8:00 am – 5:00 pm EST). The position offers a $5,000 sign‑on bonus. Responsibilities Conduct behavioral assessments to identify member knowledge, skills, and behavioral needs. Coordinate specific health coaching as directed by the nurse case manager to address objectives and goals. Implement coaching plans using behavior change principles to identify member barriers and develop ways to overcome them. Provide feedback to the nurse case manager on member goal attainment and clinical issues. Monitor and evaluate interventions, modify plans as needed. Minimum Requirements BA/BS in a relevant field (Health Education, Exercise Physiology, Respiratory Therapy, Dietetics) and at least 3 years of related experience, or an equivalent combination of education and experience. Certification, advanced certification, and/or license appropriate to the field of specialty as required. Preferred Capabilities, Skills, and Experiences Prior experience in health coaching, disease management, and knowledge of behavioral and clinical assessment techniques. Understanding of disease management principles. Salary, Location, and Benefits Locations: Massachusetts (Dedham, MA). Salary range: $68,904 – $103,356.
