GHR Healthcare is seeking a Physician Assistant Home Health for a per diem / prn job in Richmond, Virginia.
• Accepted NP specialties: AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner), Physician Assistants • Minimum 1 year of independent Nurse Practitioner experience (new graduates not accepted) Nurse Practitioner Responsibilities • Conduct annual in-home Health Risk Assessments for geriatric patients (1:1 visits, no follow-up care required) • Perform physical exams, medication reviews, and evaluate social/environmental factors • Complete basic labs (e.g., A1C, fit tests – no blood draws) • Document and submit assessments via provided tablet (same-day submission required) • Adapt to various home environments and manage logistical unpredictability (route changes, no-shows, cancellations) • Maintain high-quality standards and timely documentation • Communicate effectively with patients and the clinical support team
