Job Description Provides pharmaceutical care to patients of all ages through medication order review, preparation, dispensing, documentation, monitoring, patient counseling, and medication reconciliation. Collaborates with patients, physicians, nurses, pharmacists, and other healthcare professionals to deliver safe, effective, and fiscally responsible patient-centered care. Interprets, reviews, and processes medication orders, screens orders for appropriateness and intervenes as warranted. Provides patients and providers with therapeutic assessments and selected disease management for assigned clinical areas, working toward improving patient and practice outcomes. Estimated pay range: $65.25 / hour depending on shift as applicable. Qualifications Degrees: Bachelor's. Licenses & Certifications: Registered Pharmacist. Additional Qualifications: Bachelor of Science required. Doctor of Pharmacy (PharmD) preferred. Preferred Progressive pharmaceutical care experience (or) Pharmacy Practice Residency.
