Avera is looking for a RN –Case Manager to join our Team!

$15,000 sign-on bonus!

 

Position Highlights:

  • May be eligible for $15,000 sign-on bonus with a three-year commitment!

Job Summary

  • The Case Manager is accountable for collaborating with the Multidisciplinary Care Team to promote quality patient care. Facilitates efficient utilization of resources.
  • Identifies patient care needs throughout the continuum of care.
  • In coordination of care through collaborative practice – has excellent communication skills.
  • Knowledgeable of the Health Care Continuum and today’s challenge of change.
  • Staff is required to maintain a level of physical/mental health needed to perform their duties effectively.
  • Adheres to the Nursing Code of Ethics, St. Mary’s Healthcare Center’s Mission, Service Excellence Standards and the standards outlined in the CHI “Ethics at Work” booklet.

Education and/or Experience

  • Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.

Licensure/Certification/Registration

  • Licensed as a Registered Nurse in the State of South Dakota and/or meeting the requirements of Multi-state licensure required.

  Work Schedule

  • Monday – Friday, 8:00am-4:30pm; 80 hours every 2 weeks
    • Holidays: on-call rotation
    • On-call: every 6 th weekend/night

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

  • $29.50 - $44.25 per hour
