, permanent position Position: Inpatient Social Work Case Manager Direct Recruit Agency is currently seeking a highly motivated and experienced Inpatient Social Work Case Manager to join our team. This is a full time, permanent position with a competitive salary and benefits package. As an Inpatient Social Work Case Manager, you will be responsible for providing comprehensive case management services to patients in an inpatient setting. This may include conducting assessments, developing treatment plans, coordinating care with other healthcare professionals, and advocating for patients' needs. Key Responsibilities: - Conduct initial assessments of patients' psychosocial needs and develop individualized treatment plans - Coordinate with healthcare team to ensure effective communication and collaboration in patient care - Provide counseling and support to patients and their families, addressing any emotional, social, or financial concerns - Advocate for patients' needs and assist with navigating the healthcare system - Monitor and evaluate patients' progress and adjust treatment plans as needed - Collaborate with community resources to provide patients with appropriate support services - Maintain accurate and timely documentation of all case management activities - Stay up-to-date on relevant laws and regulations related to patient care and social work practice Qualifications: - Bachelor's or Master's degree in Social Work from an accredited institution - Current state licensure as a Social Worker - Minimum of 2 years experience in a healthcare setting, preferably in an inpatient setting - Strong understanding of psychosocial issues and ability to provide effective counseling and support - Excellent communication and interpersonal skills - Ability to work independently and as part of a multidisciplinary team - Knowledge of community resources and ability to coordinate care with other providers - Proficient in documentation and record keeping If you are a dedicated and compassionate social worker with a passion for providing quality care to patients, we encourage you to apply for this exciting opportunity. Idea Candidate Will accept LMSW or LCSW-C. Must have experience with discharge planning in an inpatient/hospital setting position working with a general MEDICAL inpatient population. FYI: This is not a mental health/psych position.
