Job Summary:
Our hospice agency is seeking a Weekend Hospice Intake/Scheduling Coordinator, a role that combines scheduling and intake responsibilities, to join our team. The successful candidate will be highly organized and detail-oriented, ensuring the efficient processing of incoming clients through our intake program while managing scheduling tasks within a hospice care setting.
Schedule: Thursday through Monday (Saturday and Sunday remote)
Responsibilities:
- Manage the intake process for all incoming referrals and client information for hospice services
- Conduct initial client screenings in a compassionate and supportive manner
- Handle client data and maintain accurate information within our hospice database with sensitivity
- Provide empathetic assistance and responses to client inquiries
- Coordinate scheduling of appointments and services for hospice clients
- Maintain regular communication with internal team members on client progress in hospice care
- Ensure compliance with HIPAA regulations and organizational policies relevant to hospice agencies
